At Lee and Murrin Endodontics, we make every effort to provide you with the finest endodontic care and the most convenient financial options. To accomplish this we work hand in hand with you to maximize your insurance reimbursement for covered procedures. Please understand that insurances policies are a contract between the policyholder and the insurance company. We accept most insurance plans, but we are contracted with the following insurance companies:
- Delta Dental Premier
- Delta Dental PPO
- Cigna Total
- Sun Life Financial
- United Concordia
- Mutual of Omaha
- Anthem Blue Cross/Blue Shield
- The Standard
- Lincoln Dental
We routinely provide treatment for patients as out-of-network providers. Often out-of-network coverage is the same as in-network coverage for specialty endodontic services. We are here to help you and are happy to answer any questions you may have. If you are out of your insurance network, please remember that there is a reason that your dentist has referred you to Lee and Murrin Endodontics and that is because they want you to have to best endodontic care possible. Even if your carrier states that it will cover the majority of endodontic fees, several factors can reduce the actual benefit you receive. These factors include the following: deductibles, annual limits and maximum allowable amounts per procedure.

Fee Policy & Dental Insurance
The fee for endodontic treatment is determined by the complexity of the specific tooth being treated. If after endodontic evaluation, treatment is determined to be necessary, a fee will be quoted prior to treatment. This fee includes all necessary radiographs (dental xrays) and office checks and recalls. In payment for endodontic services rendered we accept VISA, Mastercard, American Express, Discover, CareCredit, cash, and personal check.
Payment options are as follows:
- Fee reduction of 5% for payment in full at the time of service. We electronically submit your insurance claim the same day and benefits are sent directly to you
- 50% initial payment at time of service – we electronically submit your insurance claim the same day. After benefits are received in our office, we send a statement for the balance or a reimbursement check (whichever is applicable).
- 50% initial payment at time of service, with balance due in 30 days if no insurance coverage.
For evaluations and consultations, payment is requested at the time of service, with 5% fee reduction. This endodontic specialty practice is not a participating provider with any specific insurance plans.
